Plum Tree and the Centre for Family Medicine are pleased to announce the collaborative creation of a primary care Seniors Clinic. It will be nurse practitioner-led and provide full service primary care to people age 65+. Other health professionals supporting the multi-disciplinary team include family physicians, nursing, and pharmacy and will be covered by OHIP. The office is co-located at 16 Andrew Street, Kitchener with Plum Tree Memory Care and the CFFM Regional Memory Clinic.
